INDEPENDENT LIVING TRIALS
Flexible Durations
Our 12 Week Intensive Independent Living Trial Assessment provides opportunity for evidence gathering and recommendations for longer term needs. Generally our ILT packages vary between 6 months and 18 months, with tapering of support as skills develop and strategies are habituated. Our dedicated ILT Coordinator takes care of the process from beginning to end.
Our Triphasic Approach
This approach is designed to gradually enhance the self-sufficiency of our clients, moving them from intensive support to independent problem-solving and self-awareness.
Each phase builds upon the achievements of the previous one, in a concerted effort to promote cognitive rehabilitation and gain functional skills.
Robust Data and Evidence
Our personalised independent living trial specific reports are tailored to ensure that all elements of the trial are evidenced thoroughly. Our ILT teams undertake enhanced training for these packages, specifically around observation skills and cognitive strategy implementation. We provide regular update reports in addition to the daily logs, and our ILT Coordinator oversees the package, as well as producing an extensive final ILT report.
Case Study
Glen* is a determined 29-year-old man, whose life was dramatically altered by a traumatic brain injury resulting from a car accident 3 years ago. Despite living with his parents, Glen had strong aspirations for increased independence and dreams of setting up his own home. He had received support prior to his independent living assessment, but there had been a large gap in provision. The multidisciplinary team was eager to verify Glen’s capacity to live independently and assess the amount of support he would require to do so, and to assess future support provision needs. Following a period of assessments by BIS Services in his home with his family, and current MDT, Glen joined our ILT program in April 2023, marking the start of his pursuit of independence.
Outcomes
Baseline Improvements
Glen’s baseline assessments demonstrated significant improvement in extended activities of daily living, as measured by the FIM-FAM measure of independence. This improvement was observed across self-report, CRA observations, and parent observations both before and after the ILT. Notably, the CRAs reported the most significant improvements.
Safety and Independence
Glen successfully tested a new seizure monitor device, providing reassurance of safety and independence for him and his family.
Evidence from a loan provider’s cold calling (real life scenario fake practice), demonstrates a decrease in impulsive behaviour and an improvement in financial skills.
Sleep Quality and Fatigue
Glen experienced improved sleep quality and took more responsibility for managing fatigue as the program continued.
Glen effectively internalised the fatigue management strategies and continued to use them even after support was reduced. This lead to improved problem solving skills, reducing reliance on support.
Commonly Asked ILT Questions
How do I make an enquiry about an independent living trial?
Please complete the form below, or email Cam McIntosh here. Alternatively you can give us a call on 01622 584456
Can I see a sample report?
Absolutely, please contact us above and we would be happy to discuss.
Do you have a minimum duration?
Our 12 week assessments are the minimum duration. Most ILTs tend to run for 6-18 months dependent on the clients needs.
How do you match CRAs for an ILT?
We have a core ILT team, who are trained specifically in this area, although this is expanding as more locations are covered. We still aim to match to client needs, so for example if a team was required to be all male, or of a particular religion for cultural needs, we would recruit a new team and train them accordingly.
How do you provide evidence for the team?
Our bespoke MACHforms are distributed to the team daily (if required) and we provide interim reports every 4 weeks and run MDT meetings at regular intervals. We produce a report at the end of each assessment of evidence of intervention and recommendations for ongoing support needs.
Where do you provide ILTs?
We can provide ILTs anywhere, although more remote locations will take longer to commence due to potential recruitment needs.
Can you make recommendations for technology?
Absolutely. When we carry out our initial assessment visits we would be happy to provide input.
”….I would like to thank Cam McIntosh for outstanding work on this case and the leadership that he has shown. Having two failed ILTs behind our client, along with the issues with family/friends/insight etc, this was always going to be a challenge to get the balance right for the client.
The work with the CRA’s, communication between the team and Case Management has all been seamless. At all times you have been responsive and supportive..’.
”….Our client had a 12 week independent living trial. I found the team were all very professional, with excellent interpersonal skills and keen to work with all of the MDT in a collaborative way.
They implemented everything suggested, providing lots of helpful insight, their own ideas and feedback for ongoing needs following the placement. A very positive experience and one I’d recommend to others!…”
